Stumble/Miss at throttle tip in

Has anyone had experience with a 2013+ 6.7 having a miss at throttle tip in? This one is bone stock and will do it EVERY time if you let it come back down to an idle. Once it gets through the miss/stumble, it will not do it again until the engine returns to an idle.
The other conundrum is that under derate it will not do it either. The truck came in with the turbo actuator bad. Once i fixed that, this miss came about. I have swapped FCA, EGR valve, MAP sensor, and i have unplugged the intake throttle valve. All with no change, except when it went into derate because of the throttle valve being unplugged.
It's like a rail pressure problem in that it really rattles like a rail pressure spike after it pushes through the miss/stumble. But graphs on the scantool show nothing concerning.

It now has set a p026b injection timing performance. Have a couple vids of what it's doing.

Ended up being bad injectors. Just about every 2014 we get in has bad injectors if it has some miles on it. That p026b code is a telltale sign of bad injectors most times
