Top right.

To the left and below the cam. The one closest to the cam is 12V, the one a little lower is CR.

Just use a 12V back plate as a template, you can do this by hand or on a drill press. It's not like the bolts fit the holes all that tight, and the dowel sleeves line it up anyhow.
