3rd time breaking a counter shaft.

truck is a 02 ext cab short bed 2500, 24 valve, 5 speed trans, 3:55 gears. running ats 4k/5k twin set up, 240hp injectors, smarty/edge combo, 1 3/8th input shaft, haisley street puller clutch, as well as traction bars. truck has broken counter shaft in trans 3 times now, and stripped 5th gear once. what do you guys think is breaking this shaft in the trans? been hearing everything from engine out of balance, bell housing out of round, drive shafts not balanced, rear end locking/unlocking while driving causing stress in drive-line, trans housing torqued, and it has done it with 33 inch mud tires on 18inch wheels, and stock size all terrains on stock wheels as well. some more input would be great, thanks!
 
case flex with sn nv4500 is unheard of the cases are cast iron if it flexs it would only be .005 max imo and it would take a 1200hp truck, pulling a sled... on black top to get it to flex that much! btw your avrage sheet of computer paper is .003 thick
 
I have broke 3 c/s 4 sets of 5th gears 9 5th synchro hubs and finally teeth went on the input so I just bought a nv5600 and haven't had it break yet$.02
