A couple of things that have me puzzled..

I recently installed a known working, driving, used VP pump. Truck ran perfect. No issues. And didn't throw any codes or hidden codes. Within the past week I have developed a hard start issue. That issue has now become a run perfect when cold in the morning, and then die when put into gear after each restart. What causes the 'die when put into gear'?

I do have a 0216 code that doesn't trip the CEL, but shows up on a reader.

Ideas? I have searched, but nothing with this particular issue. Just found hard start stuff.

To late to edit:

I have done the APPS reset. And, I have checked voltage. APPS voltage called for .628, voltage at the ECM plug was .88. I managed to get it to .66.

I have checked all ground locations. And supply lines for fuel. I can't find a leak.

What I have noticed is that the truck has an in-tank pump installed via a dealer before I bought it. The in-tank pump supplies fuel to the factory lift pump. Then, the lift pump to the filter canister.
 
I haven't checked yet. That is the chore for tomorrow seeing how all of my tools are still at work.

Meanwhile, I have found plenty of posts saying there is a schroeder valve on the side of the pump for the 01 and up. I haven't been able to find one in regards to the proper test location for the 99 model trucks. Can someone please point me in the proper direction. I have the fuel pressure tester already.

On top of the FF are 2 - 1/8"npt ports. the one closest to the center on the left is the one to put the gauge in.
 
Had to buy some schroeder valve attachments to make my gauge work.

Idle: 5psi (gauge goes from 0 to 10, it splits it)
Manual rev from under hood in park: 3.5psi
Backed down my main drag of dirt road, and holding brake to put engine under load: 3-3.5 psi
WOT on dirt, with tire spin: 0-2 psi

Lift pump is in order.
 
Well, I'm going to limp on a new lift pump for as long as I can first. Business isn't picking up at work, and neither is my paycheck. :doh:
