Busted Piston issues?

The shop found 2 busted pistons today #1 and #6 in the ring land area about 2' length . I'v read this b4 but never looked into why or this common ????
 
Seized from heat? Were they on the front side of #1 and the rear side of #6?
 
I've seen a few now with cracked ring lands below the second ring, these were the 04.5 pistons though.

One was from bind, the other combustion pressure broke it after the first ring failed.
 
It wasnt seized I'v been drving the truck but during mid season last year I noticed some blowby but my last pull it was real bad, It would blow the oil cap into my hand with a little woosh when I took it off so we decided to put rings/bearings in and found these and they were below the 2nd ring ,, also last night I was thinking about this when I had the engine built in 06 one of the pistons was busted during shipment I guess I'm wondering if any more may have been stressed
 
i have done inframes on three c series bus engines in the past 2 months, each engine had busted ring lands on the oil control ring... turned out one of the shop helpers was starting them with either..... on an engine with grid heaters... is was always #3 piston.
