i just installed a new spicer 210391-1x carrier bearing on my driveshaft and still have a vibration all u-joints look and feel good . Has anyone that installed one of these carrier bearings had to modify the bearing or mount in anyway to install it ?

I modified my Carrier bearing with a one pcs alum. drive shaft from an 08!:)
no more vibration.

I didn't have to do anything with mine when I replaced it with that Spicer bearing. Might check the driveshaft and see if there is a weld spot where a weight might have fallen off.

Ditto^^^^^ Have it balanced also make sure your pinion and or trans output are secure!
Or Put it on stands and spin it at 55 to see exactly what is going on! CAUTION!!
Please use common sense and help when doing this!

Changing the bearing. I have been trying to chase down a vibration, and while this is not what is causing the vibration I am looking for, the bearing has gone bad. It's completely seperated from the rubber.

Is the drive shafts lined up properly yoke to yoke.

I imagine its failed because my truck has 215k on it. I don't have a lift installed, and have not done anything that would have changed the factory angles. However I did have the (auto)trans rebuilt about 20k ago (trans cooler went out and killed it)... would that possibly throw it out of alignment when they reinstalled it? The vibration I am trying to track down is more audibal than felt, only in drive, and only below 800 rpms. I just noticed the carrier bearing because there is also a vibration between 65-70 and I saw that it can be the culprit for that one.
