Need help with sluggish truck

My brother has a 98.5 dodge 2500 2wd 5 speed that is bone stock and the truck has no power what so ever. If it's In neutral it will rev up fine but when u drive it it that for ever to gain rpms and not sure y.u can stomp the truck to the floor in 2 and take for ever to hit 2 grand and it like that in all gears . I checked fuel pressure and it fine not sure what else to check any help would b great
 
go somewhere and get the codes pulled. mine has been doing the same thing lately when my box is turned off. It took me 3 miles to get up to 55mph. I have a 0216 code which is one of the pump death codes but i can still drive it ok with the box turned up.
does it white smoke really bad when you first try to drive it?
 
Yea it could be a dying vp or it could be in limp mode. If the check engine light is on, it may be in limp mode. If its the vp, it might have a p0216 code. When my 2000's vp was dying it blew out lots of white smoke from the retarded timing. It never showed the p0216 code though.
 
What does it do when you rev the truck in neutral? I could hold mine on the floor in nuetral and it would slowly rev like you are describing, but it would run rough and blow white/blue smoke like crazy. I would bet its probably your vp dying.
 
The truck rev fast in neutral like nothing is wrong it only slow revving when ur driving and no smoke at except when u first start it it's a little cloud of black smoke
 
I just noticed that the truck get real jumpy in 4 and 5 gear like it is runing out fuel and then picking back up and it use to do the same thing when it had a auto but it would only do it in drive
 
It also could have been a intercooler boot came loose and pop off so you are basically driving without a turbo
