Part Number for 12 valve t-stat housing

We're working on a Cummins conversion in a 96 Hummer H1 and need to find a part number for the 12 valve thermostat housing that has the water outlet sticking straight up. Does anybody know what it is or where I can find one??? Thanks!

Try 3914409 it goes straight up . Its not one that has a mount for the dodge alt though it has a pad for a different style alt bracket . (a GM alt) the alt bracket is pn 3910713 it uses a spacer pn 3910715 this should allow the use of a GM 10si alt which will require the pulley pn 3908560 and pulley retainer 3908563 (8 grove) . If my numbers are right it should look like this

This is with a 94 6.5 alt You would need to fab a lower tab for the alt though.Rich
