South bend 4200 DD clutch

I have a used South bend 4200 DD. It is out of my 02 dodge with a 5 speed. It also takes the big input shaft. I can text or email pic of it. Still has some pad wear left on it and the center plate is in good shape. Asking $500

What kind of life would you say is left in it? Does it need a starter spacer? Fit in a 94 NV4500 with big input? Come with flywheel?

Probably 1/3 or better life. Yes it is complete. Does not take starter spacer.
