04.5 fan and clutch assembly.

This one has the clutch wire bracket near the crank position sensor. I know it's different than my 03. Not sure what all years it'll fit. Has a corner rounded on one of the blades as you can see in the picture. I paid $185 for it from a salvage yard in the exact condition it's in. I'll ship it for $185. Warranty ended up covering mine.

I went to dodge today. That was a waste of time. They said it "should" fit 04.5 and up. My suggestion is for you to look under your fan. If the fan clutch wire connects to the harness right near the crank position sensor than it will work on your 06.
