Need some help here, what does this appear to be or caused by?

What are these from and what kind of application were these subject to? What material?
 
flat bottom hole, what does the other end of the bolt look like (the broken piece)
 
I'd like to know a little more about what kind of joint was being bolted here, the grade of the bolt, how much torque was put to it, and how long it's been in service.

Typically, cup-and-cone fractures are associated with straight up ductile fracture from simple overload, but that's a really simplistic analysis without much info to go on.
 
They are flex plate bolts on a 7.3, all 10 did the same thing. They are original(2002)factory 10.9 bolts torqued to 100lbs. Been torqued approximately 6 different times over the last 7 years.
 
The 'Keith Black of diesel motors' makes too much power and no bolt is strong enough?
 
The bolts lasted longer than the first 5 motors, I'd say they did good! LOL
